GRAMA requests are for existing records—not questions, explanations, or newly created documents. A specific request helps the District identify and locate responsive records efficiently.

Do not include Social Security numbers, payment-card details, medical information, identification documents, or unrelated sensitive information.
Use Contact for general questions →Use Utah State Archives' model GRAMA request form to prepare a written request. Complete the PDF, save a copy, and send it to Hideout Local District #1 by email or mail.

Provide your contact information and describe the existing records with enough detail for the District to identify them. Depending on your PDF reader, you may complete it electronically or print it.
Save the completed PDF and attach it to an email, or print and mail it to the District. Keep a copy of the completed request and your delivery record.

The District uses the information you provide to receive, evaluate, route, and respond to your records request. Required information is needed to process a complete written request; without it, the District may be unable to identify the request or communicate its response. Information may be shared with District personnel, legal counsel, service providers, or other governmental entities as needed to process the request or as required by law. GRAMA requests and related correspondence may themselves be public records. They are generally retained for two years after final action under Records Access Requests and Appeals (GRS-1711), subject to legal holds or other applicable requirements.
